The Big Bad was once a poor scientist, attempting to do good as best he could for the city. He played his part as a doctor and patched people up for some money, but his passion was in experimentation and fighting disease and sickness at a more basic level. Over the years, he became aware of a strange sickness that periodically infested the poor population, then died out. Investigating, he soon became obsessed with the epidemic. Eventually, he found that the sickness came from growths within the complex and overrun sewer network where he collected mold growths for his experiments. Soon he was spending all of his time in the dank underground, experimenting on any specimen he could find and searching for the cure.
As time went by, his self-professed cures repeatedly failed. Nothing he could make worked, and when something showed the slightest success, it was taken by the more well-known and corrupt scientists immediately. Soon he became far more interested in his research than the people that the research was for.
Many years later, he was a deranged madman, living permanently in the sewers and conducting mad experiments on the hobos and homeless inhabitants of the sewers. He was repeatedly drawn to a section of sewer underneath the Industrial Sector, where the best mold grew, fed upon by a sort of sewer rabbit. Repeated exposure to radioactive waste there soon gave him psychic insight into the inhabitants of the sewer.
And from there, the story is obvious. He grew slightly more insane, but remained relatively stable. His experimental subjects became his minions, and his laboratory became his domain. He grew into the background of the Capital City, another patch in its remarkable tapestry. And there he stayed.
